In this role, you will provide critical support to patients and their families by addressing psychosocial needs, offering counseling, and connecting clients with appropriate community resources. Requirements
- Master's degree in Social Work (MSW) from an accredited program
- Current licensure as a Social Worker (LMSW or LCSW) in the state of practice
- Experience in home health, hospice, or related healthcare settings is preferred
- Strong assessment, counseling, and case management skills
- Excellent communication and interpersonal abilities
- Valid driver's license, reliable transportation, and car insurance
Additional requirements may include successful completion of background checks and drug screenings as per company policy.
