Job Title: Medical Social Worker (MSW) – PACE Program

Job Type: Temporary (6-Month Assignment)
Schedule: Monday–Friday, 8:00 AM – 5:00 PM
Compensation: Competitive hourly pay (based on experience)


Position Summary

We are seeking a compassionate and experienced Medical Social Worker (MSW) to provide comprehensive case management and psychosocial support for participants enrolled in a PACE (Program of All-Inclusive Care for the Elderly) program. This role is responsible for conducting participant assessments, coordinating care, providing counseling, and connecting participants and their families with appropriate community resources.

The position requires regular travel for participant home visits and in-home assessments within the local service area. The ideal candidate is committed to delivering person-centered care while collaborating with an interdisciplinary healthcare team.


Key Responsibilities

  • Provide direct social work services and case management to program participants.

  • Conduct psychosocial assessments, screenings, and ongoing evaluations.

  • Develop, implement, and monitor individualized care plans.

  • Provide individual and family counseling, crisis intervention, and emotional support.

  • Coordinate referrals to community agencies and support services.

  • Conduct home visits and in-home assessments as required.

  • Collaborate with physicians, nurses, therapists, and other interdisciplinary team members to ensure coordinated care.

  • Maintain accurate, timely, and compliant documentation.

  • Ensure adherence to organizational policies, regulatory standards, and applicable state and federal requirements.

  • Advocate for participants to promote independence, safety, and quality of life.


Minimum Qualifications

  • Master's Degree in Social Work (MSW) from an accredited School of Social Work.

  • Valid driver's license and reliable, insured personal vehicle.

  • Ability to travel regularly for participant home visits.


Preferred Qualifications

  • Experience providing case management in healthcare, geriatrics, home health, PACE, hospice, or community-based programs.

  • Knowledge of counseling theories, intervention techniques, and crisis management.

  • Strong organizational, documentation, and project management skills.

  • Excellent communication and interpersonal skills.

  • Bilingual proficiency in English and Spanish preferred.
