    Position Overview

    Neighborhood Health Association is seeking a motivated, team player to develop creative and measurable ways to engage pregnant women in prenatal health care who are resistant to or are late entries into such care.

    Key Responsibilities

    Patient Outreach & Care Coordination

    • Utilize the Patient Record System (PRS) and Electronic Health Record (EHR) to identify pregnant patients who are due for appointments or require follow-up care.
    • Conduct outreach through phone calls, letters, text messages, and other communication methods to encourage timely engagement in prenatal and pregnancy-related care.
    • Provide in-person patient support and care coordination when needed.
    • Assist patients in identifying and overcoming barriers to care, including transportation, scheduling, childcare, financial concerns, and access to community resources.
    • Support patients in attending referral appointments when necessary.
    • Document patient interactions and care coordination activities accurately in the EHR.
    • Communicate patient needs and concerns appropriately with healthcare providers and interdisciplinary care teams.

    Community Engagement & Program Development

    • Develop creative, measurable outreach strategies to increase early entry into prenatal care.
    • Build and maintain relationships with community partners and participate in community events to promote maternal health services.
    • Participate in community meetings, committees, and collaborative projects related to maternal and child health.
    • Assist with the development, implementation, and evaluation of maternal health programs and initiatives.

    Data Collection, Reporting & Quality Improvement

    • Collaborate with the Quality Improvement Director to establish weekly and monthly patient engagement goals.
    • Collect, track, and analyze outreach and engagement data to monitor program effectiveness.
    • Prepare monthly reports on outreach activities, engagement outcomes, and performance measures.
    • Compile and submit data and reports required by funding agencies and regulatory entities.
    • Support quality improvement initiatives aimed at enhancing maternal healthcare outcomes.

    Administrative & Organizational Support

    • Assist senior leadership, the Medical Clinic Coordinator, Quality Improvement Director, and other team members with administrative functions related to maternal health programs.
    • Support organizational projects, including EHR upgrades, clinical training initiatives, competency tracking, and workflow improvements.
    • Attend staff meetings, trainings, and organizational meetings as required.
    • Perform other duties as assigned.

    • Maintain professional appearance and adherence to organizational dress code standards.
    • Demonstrate flexibility in work assignments and support departmental staffing needs.
    • Regular and reliable attendance at assigned work locations is an essential function of this position.

    Education

    • Certificate or diploma from an accredited Medical Assistant (MA) program or Licensed Practical Nurse (LPN) program required.
    • Current Basic Life Support (BLS) certification required.

    Job Requirements/Skills

    Experience

    • Minimum of two (2) years of supervisory or management experience required.
    • Experience in case management required.
    • Experience working in a community health center, Federally Qualified Health Center (FQHC), or similar healthcare setting preferred.
    • Experience in program development and implementation within a community-based organization preferred.
    • Experience utilizing Electronic Health Records (EHR) systems and healthcare databases.

    Knowledge & Competencies

    • Knowledge of principles and best practices related to program design, development, implementation, and evaluation.
    • Strong understanding of care coordination, patient engagement, and community resource navigation.
    • Ability to analyze data and use metrics to drive program improvement and patient outcomes.
    • Experience with event planning, curriculum development, and community outreach initiatives.
    • Proficiency in Microsoft Office Suite and Windows-based computer applications.
    • Strong organizational skills with the ability to manage multiple projects and priorities simultaneously.
    • Excellent interpersonal, verbal, written, and presentation skills.
    • Ability to communicate effectively with patients, community partners, leadership, and multidisciplinary teams.
    • Demonstrated cultural sensitivity and ability to work effectively with diverse populations.
    • Flexible, adaptable, and able to respond effectively to changing organizational needs.
    • Strong problem-solving, strategic thinking, and analytical skills.
    • Commitment to maintaining confidentiality, professionalism, and ethical standards.
    • This is a full-time, exempt salary position, Monday - Friday, no weekends or holidays
