If you are looking to make a difference in the lives and families that we serve, we are looking for a full-time (40 hours/per week and benefit eligible) MDS Care Plan Coordinator to join our skilled workforce in our skilled nursing facility setting.
The MDS Care Plan Coordinator Job Responsibilities:
· Completes an admission care plan for the residents.
· Determines the Assessment Reference Date at time of admission.
· Advices the unit of the “look back” period and provides the unit with the Certified Nursing Assistant worksheet.
· The Manager on Duty will do facility rounds, checking each department and getting reports from the nursing supervisors. Duties will include, but are not limited to, facility tours for prospective admissions, assisting with resolution of resident concerns and overall facility operation.
· All other duties assigned.
Qualified applicants will have the following:
· Must have an active Florida Registered Nurse License, BSN.
· Minimum, three (3) years’ experience in a skilled nursing facility (SNF).
· Point Click Care experience preferred.
· Must know basic data entry, MDS software experience preferred
· Must be able to read, write, speak and understand the English language and possess good communication skills.
· Must possess the ability to make independent decisions when circumstances warrant such action.
· Must possess the ability to handle tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public.
· Must be able to work harmoniously with other personnel.
· Must possess the ability to plan, organize, develop, implement, and interpret the programs, goals, objectives, policies and procedures, etc. that are necessary for providing sound accounting techniques.
· Must be able to carry out and understand written and oral instructions.
· Must possess the ability to examine and verify financial documents and reports.
Working Conditions:
· Works in office areas as well as throughout the facility
· Ability to sit for time required to complete paperwork duties required
· Able to be mobile which includes lifting up to 20 lbs. of force or force frequently.
· Essential physical demands include: lifting, climbing, bending, pushing, standing or sitting, reaching, hearing and speaking. However, reasonable accommodations may be made to enable qualified individuals with disabilities to perform essential functions.
· Subject to hostile and emotionally upset residents, family members, personnel and visitors.
