The Opportunity:
We're looking for a detail-oriented MDS Coordinator to join us on a full-time basis. In this role, you'll ensure accurate and timely MDS assessments, review PDPM classifications, and support staff with up-to-date Medicare guidelines. Key Responsibilities:
Review clinical documentation and hospital records to ensure completeness and compliance with CMS regulations.
Review and validate Patient-Driven Payment Model (PDPM) information for accuracy and reimbursement optimization.
Evaluate the quality of care being provided to long-term care residents through detailed assessment and documentation.
Complete MDS assessments and ensure their accuracy and timely submission in compliance with state and federal regulations.
Maintain up-to-date knowledge of Medicare and Medicaid criteria and serve as a reliable resource for nursing staff.
Communicate regulatory changes and provide guidance and training as needed.
Monitor Medicare assessment schedules and nursing documentation to ensure consistency and compliance.
Qualifications:
Registered Nurse (RN) or Licensed Practical Nurse (LPN) with a current state license.
Minimum of 2 years of experience in long-term care, preferably with MDS coordination responsibilities.
Strong knowledge of MDS 3.0, PDPM, and CMS guidelines.
Detail-oriented with excellent analytical, organizational, and communication skills.
Proficient in electronic medical records and MDS software.
Ability to work independently and collaborate with interdisciplinary teams.
Industry standards applicable to the state depending on work experience and level of expertise.

Salary Range: $90K to $97K and other benefits
