Position Summary

The MDS Coordinator is responsible for completing resident assessments, developing care plans, and ensuring accurate reimbursement in compliance with Medicare and Medicaid guidelines.

Key Responsibilities
  • Complete and manage resident MDS assessments, including CAAs and triggers
  • Coordinate interdisciplinary care plan meetings and ensure timely updates
  • Develop and revise individualized care plans based on resident needs
  • Support Medicare/Medicaid reimbursement processes and ensure accurate RUG coding
  • Review clinical documentation to ensure compliance and proper billing
  • Lead interdisciplinary care plan team meetings
  • Conduct audits to ensure accuracy, timeliness, and compliance of MDS documentation
  • Support survey readiness and respond to regulatory requests
  • Train staff on RAI process and documentation requirements
  • Maintain compliance with all federal, state, and facility policies
Qualifications
  • Registered Nurse (RN) or Licensed Practical Nurse (LPN) per state/facility requirements
  • Completion of an accredited nursing program
  • MDS, care planning, and assessment experience preferred (long-term care experience strongly preferred)
  • Strong attention to detail and knowledge of Medicare/Medicaid guidelines
  • Excellent communication, organization, and documentation skills
  • Ability to work collaboratively with interdisciplinary teams
Additional Requirements
  • Ability to lift up to 75 lbs and perform physical tasks as needed
  • Must meet all state and federal regulatory requirements
  • Strong interpersonal skills with energy, professionalism, and teamwork focus
