In this specialized role, you'll help ensure accurate assessments, regulatory compliance, and coordinated care planning while partnering with an interdisciplinary team focused on achieving the best possible outcomes for patients.

What We're Looking For:
Associate Degree in Nursing (ADN) required
Bachelor's Degree or BSN preferred
Current RN license in the applicable state required
Two years of experience in an applicable care setting required
MDS experience preferred
RAC-CT certification preferred
Strong knowledge of MDS, Medicare, reimbursement, and regulatory requirements
Exceptional organizational, communication, and critical-thinking skills
Ability to work collaboratively across disciplines and serve as an educational resource to team members
Commitment to continuous learning and professional development
What You'll Do:
Coordinate and complete Minimum Data Set (MDS) assessments accurately and within required timeframes
Oversee the Resident Assessment Instrument (RAI) process
Develop and coordinate interdisciplinary care plans that support individualized patient needs
Ensure compliance with Medicare, state, and federal regulatory requirements
Support pre-authorization, reimbursement, and utilization management processes
Monitor documentation and assessment accuracy to optimize quality outcomes and reimbursement
Educate and serve as a resource for staff regarding MDS, Medicare, and regulatory requirements
Collaborate with physicians, nursing staff, rehabilitation teams, case management, and discharge planning teams
Assist with coverage and departmental support as needed, including a 7:00 a.m. to 3:30 p.m. schedule when required
