MDS Coordinator (LPN)Agawam West Rehab & Nursing is seeking an experienced LPN for a full-time, salaried MDS Coordinator position.The MDS Coordinator will oversee the coordination and completion of resident assessments, ensuring accurate clinical documentation, timely MDS completion, and appropriate reimbursement. This role works closely with nursing, therapy, and the interdisciplinary team to maintain accurate and comprehensive resident assessments.ResponsibilitiesCoordinate, schedule, and complete MDS assessmentsCollect and review clinical information from nursing, therapy, and medical recordsEnsure accurate and timely documentation and submissionsParticipate in interdisciplinary care planning and resident assessment reviewsSupport regulatory compliance and accurate reimbursementMaintain knowledge of current MDS and CMS requirementsQualificationsActive Massachusetts LPN license requiredMDS experience in a skilled nursing facility requiredStrong understanding of the MDS process, including coordination, scheduling, and completionLong-term care or rehabilitation experienceStrong organizational and attention-to-detail skillsCompensation & BenefitsSalary: $91,000 - $95,000 per year (DOE)Quality Measure Bonus: Up to $7,500 per yearSalary increases tied to censusMedical, dental, vision, 401(k), PTO, and more
