WE’RE HIRING: MDS COORDINATORJohn Knox Manor Nursing HomeAre you an experienced MDS Coordinator who takes pride in accuracy, compliance, and quality resident care?We’re looking for a knowledgeable, organized, and compassionate MDS Coordinator to join our team and play a key role in ensuring our residents receive the care they deserve.WHAT YOU’LL DO

• Coordinate and oversee the MDS assessment process
• Ensure assessments are accurate, timely, and compliant with current CMS requirements
• Collaborate closely with Nursing, Therapy, Social Services, Dietary, and other interdisciplinary team members
• Lead and facilitate care plan development and coordination
• Monitor documentation and identify opportunities to improve clinical outcomes
• Support regulatory compliance and survey readiness
• Help ensure the interdisciplinary team is working together toward the best outcomes for our residents
• Maintain certs/recerts
• Initiate death certificates 

WHAT WE’RE LOOKING FOR

• Previous MDS Coordinator experience in a Skilled Nursing Facility
• Strong knowledge of MDS 3.0 / RAI processes
• Understanding of Medicare, Medicaid, and SNF reimbursement requirements
• Excellent organization skills and attention to detail
• Strong communication and leadership skills
• A team player who is committed to resident-centered care
