The MDS Coordinator ensures accurate, compliant, and optimized clinical documentation and plays a key role in patient care planning and reimbursement accuracy.

Key Responsibilities
\n

Complete and submit accurate MDS assessments (admission, quarterly, annual, and significant change).
\n
Ensure proper PDPM coding and patient classification.
\n
Collaborate with interdisciplinary teams for care planning.
\n
Monitor documentation supporting clinical acuity.
\n
Assist with regulatory surveys.
\n
Educate nursing staff on the importance of accurate documentation.
\n
\n
Qualifications
\n
\n
Active RN license (KS or IA based on location).
\n
1–2 years of MDS experience in a Skilled Nursing Facility (SNF).
\n
Strong knowledge of PDPM, CMS RAI Manual and MDS Regulations.
\n
Excellent attention to detail and organizational skills.
\n
\n
