MDS COORDINATOR - DIRECT HIRE
The MDS Coordinator is responsible for coordinating and managing the completion of the Minimum Data Set (MDS) assessments in accordance with federal and state regulations. This role ensures accurate clinical assessments, compliance with CMS guidelines, appropriate reimbursement, and the development of individualized care plans that support quality resident outcomes.
Key ResponsibilitiesMDS & Clinical Assessment- Coordinate, complete, and submit accurate and timely MDS assessments (admission, quarterly, annual, significant change, and discharge).
- Ensure compliance with CMS, state, and facility regulations related to MDS and RAI processes.
- Review clinical documentation to support MDS coding accuracy and reimbursement.
- Monitor and manage ARD schedules and assessment reference dates.
Care Planning & Interdisciplinary Collaboration- Lead or participate in interdisciplinary care plan meetings.
- Collaborate with nursing, therapy, social services, dietary, and other disciplines to develop and update comprehensive care plans.
- Ensure care plans reflect current resident needs, preferences, and clinical status.
Reimbursement & Quality Measures- Monitor case mix, PDPM (or applicable payment model), and reimbursement accuracy.
- Identify clinical risks or missed opportunities affecting reimbursement.
- Monitor quality indicators and outcomes, and assist with quality improvement initiatives.
Compliance & Audits- Ensure accurate documentation to support audits and surveys.
- Participate in state and federal surveys as related to MDS and care planning.
- Maintain current knowledge of regulatory changes affecting MDS and reimbursement.
Education & Support- Educate staff on documentation requirements and MDS-related processes.
- Serve as a clinical resource for MDS, care planning, and reimbursement questions.
QualificationsRequired- Active RN license in good standing in CT
- MDS Coordinator Certification (e.g., RAC-CT, AANAC) or willingness to obtain
- Minimum of 12 years of clinical experience in long-term care or skilled nursing
- Strong knowledge of MDS 3.0, RAI process, and CMS regulations
Preferred- Prior experience as an MDS Coordinator
- Experience with PDPM reimbursement model
- EMR/EHR experience (e.g., PointClickCare, MatrixCare)
