MDS Coordinator - Registered Nurse

MDS Coordinator (RN) – Full Time, Benefit Eligible
Salary: $94,723–$99,340/year (DOE)
Location: Little Falls, MN
Schedule: Full-Time | Exempt | Day Hours

Position Summary

The MDS Coordinator is responsible for coordinating and completing the Resident Assessment Instrument (RAI) process, including the Minimum Data Set (MDS), Care Area Assessments (CAA), and care plans in accordance with state and federal regulations. This role plays a key part in ensuring quality outcomes, accurate reimbursement, and a positive resident experience. Part of the on-call RN rotation. 

Key Responsibilities

• Plan, schedule, and complete MDS assessments and submissions
• Lead interdisciplinary care planning and documentation review
• Identify and address changes in resident conditions
• Monitor Medicare eligibility and documentation
• Ensure compliance with state and federal guidelines
• Support Quality Assurance and Performance Improvement (QAPI) efforts
• Educate and support clinical staff on the MDS process

Qualifications

• Current RN license in good standing
• Prior MDS/RAI experience preferred (will train the right candidate)
• Knowledge of state and federal long-term care regulations
• Strong assessment, communication, and documentation skills
