Beaumont Health Care Center is seeking an experienced and detail-oriented Registered Nurse (RN) MDS Coordinator to join our leadership team.

This is an excellent opportunity for a skilled nursing professional who understands the critical role accurate MDS assessments, care planning, and reimbursement play in delivering quality resident care and supporting facility success.

**The ideal candidate will have experience with the MDS process, Medicare and Medicaid reimbursement, care plan development, and interdisciplinary team collaboration within a Skilled Nursing Facility.**

What You'll Do

  • MDS & Clinical Reimbursement
  • Coordinate and complete MDS assessments in accordance with federal and state regulations
  • Ensure timely and accurate completion of all MDS submissions
  • Review resident records to support appropriate clinical reimbursement
  • Monitor assessment schedules and regulatory deadlines
  • Maintain compliance with PDPM requirements and reimbursement guidelines
  • Care Planning
  • Participate in interdisciplinary care plan meetings
  • Assist in developing and updating resident-centered care plans
  • Collaborate with nursing, therapy, dietary, social services, and medical providers
  • Ensure care plans accurately reflect resident needs and services provided
  • Clinical Documentation & Compliance
  • Conduct chart reviews and audits to ensure documentation supports coding accuracy
  • Educate nursing staff regarding documentation requirements
  • Monitor quality measures and identify opportunities for improvement
  • Assist with survey readiness and regulatory compliance initiatives
  • Quality Improvement
  • Participate in Quality Assurance and Performance Improvement (QAPI) activities
  • Support efforts to improve resident outcomes and facility performance metrics
  • Track and analyze quality measures and reimbursement trends

Qualifications

Required

  • Current Texas RN license in good standing
  • Skilled Nursing Facility (SNF) experience
  • Knowledge of MDS, RAI process, and care planning
  • Strong organizational and time management skills
  • Excellent communication and interdisciplinary collaboration abilities

Preferred

  • Previous MDS Coordinator experience
  • RAC-CT certification strongly preferred
  • Experience with PDPM reimbursement
  • PointClickCare experience
  • Knowledge of Medicare, Medicaid, and CMS regulations
