MDS Coordinator (RN)

Charleston Rehabilitation and Nursing is seeking a full-time MDS Coordinator (RN) to join our skilled nursing team.

Salary starting at $90,000 annually plus a Quality Measure Bonus opportunity of up to $7,500 per year. Salary growth is tied to facility census and performance.

This position is responsible for coordinating and completing MDS assessments, supporting accurate clinical documentation, and helping ensure appropriate reimbursement and quality outcomes.

We are looking for an organized, detail-oriented Registered Nurse who is passionate about quality care and resident outcomes.

Qualifications:

• Active RN license required
• Skilled nursing or long-term care experience required
• Knowledge of skilled nursing documentation and resident assessments preferred
• MDS experience preferred
• Training and support available for the right RN with strong skilled nursing experience and an interest in MDS
