Job description MDS Coordinator Job Description Overview The MDS Coordinator plays a vital role in ensuring the accurate assessment and documentation of resident care in compliance with federal and state regulations. This position is essential for maintaining high-quality care standards within nursing homes, acute care facilities, and home care settings. The ideal candidate will possess a strong understanding of medical terminology, and ICD-10 coding, along with experience in primary care and familiarity with electronic health record systems such as Cerner. Duties Conduct comprehensive assessments of residents to develop and implement individualized care plans. Ensure compliance with all regulatory requirements related to the Minimum Data Set (MDS) assessments. Collaborate with interdisciplinary teams to coordinate resident care and optimize outcomes. Maintain accurate records of assessments, care plans, and progress notes in accordance with facility policies. Provide training and support to nursing staff on MDS processes and documentation requirements. Monitor quality indicators and participate in quality improvement initiatives. Serve as a resource for clinical staff regarding medical terminology, coding, and documentation practices. Experience Previous experience in a nursing home or acute care setting is preferred, but not required Familiarity with ICD-10 coding, and MDS assessment processes is highly desirable. Knowledge of primary care practices and home care services is a plus. Proficiency in using electronic health record systems, particularly MatrixCare. Strong communication skills to effectively collaborate with healthcare professionals across various disciplines. Ability to work independently while managing multiple priorities in a fast-paced environment.
