Lacon Rehab & Nursing is seeking an experienced MDS Coordinator RN for a full-time salaried position in our skilled nursing facility.
This role is responsible for coordinating and completing MDS assessments, ensuring accurate documentation, supporting reimbursement optimization, and collaborating with the interdisciplinary care team.
Qualifications
- Active Illinois RN license required
- MDS experience in skilled nursing or long-term care required
- Strong understanding of MDS scheduling, coordination, and completion
- Excellent assessment and documentation skills
