Position Summary:
We are seeking a detail-oriented and knowledgeable MDS Coordinator (RN or LPN) to join our team. The ideal candidate will be responsible for coordinating and completing the Minimum Data Set (MDS) assessments in accordance with federal and state regulations, ensuring accuracy in resident assessments and care planning.
 

Key Responsibilities:

  • Coordinate and complete MDS assessments and care plans in a timely manner
  • Ensure compliance with all CMS, state, and facility regulations
  • Collaborate with interdisciplinary team members to develop and update resident care plans
  • Monitor documentation to ensure accuracy and support reimbursement
  • Participate in quality assurance and performance improvement activities
  • Maintain current knowledge of MDS guidelines and regulatory changes

Qualifications:

  • Current, active RN or LPN license in the state of Texas
  • Previous MDS experience in a skilled nursing facility preferred
  • Strong understanding of CMS regulations and reimbursement systems (PDPM)
  • Excellent organizational and communication skills
  • Ability to work independently and as part of a team
