We are seeking an experienced and detail-oriented MDS Coordinator (RN or LPN) to join our skilled nursing team. This position plays a critical role in ensuring the accurate and timely completion of resident assessments, Medicare documentation, care planning, and regulatory compliance.

Responsibilities
Coordinate and manage the Resident Assessment Instrument (RAI) process.
Complete and submit MDS assessments accurately and within federal and state-required timeframes.
Ensure timely completion, data entry, lock dates, and transmission of all MDS assessments.
Monitor Medicare residents and ensure documentation accurately reflects care needs and services provided.
Review clinical records and collaborate with interdisciplinary team members to ensure accuracy of assessments and reimbursement.
Conduct Medicare meetings and participate in care plan meetings.
Maintain compliance with Medicare, Medicaid, and regulatory requirements.
Monitor RUG/PDPM reimbursement accuracy and related reporting.
Communicate effectively with nursing, therapy, dietary, social services, physicians, and other care team members.
Assist with staff education related to documentation, ADLs, restorative nursing, and assessment processes.
Maintain resident confidentiality and uphold resident rights at all times.
Qualifications
Current, unencumbered RN or LPN license.
Nurse Licensure Compact (NLC) state license preferred.
MDS experience in a Skilled Nursing Facility or Long-Term Care setting.
MDS Certification (or eligibility to obtain certification) preferred.
Strong knowledge of Medicare, Medicaid, PDPM, and RAI regulations.
Excellent organizational, communication, and leadership skills.
Proficiency with electronic medical records and MDS software.
Ability to work independently while collaborating effectively with an interdisciplinary team.
