MDS Coordinator - RN

In this role, you will:• Complete and coordinate MDS assessments in accordance with state and federal regulations• Ensure accuracy and timely submission of all MDS documentation• Work closely with nursing, therapy, and interdisciplinary teams to support resident care plans• Monitor reimbursement and regulatory compliance related to MDS processes• Assist with care planning and resident assessments

Qualifications:• Active RN license required• Minimum 2+ years of MDS experience in a skilled nursing or long-term care setting• Strong understanding of Medicare, Medicaid, and reimbursement guidelines• Knowledge of state and federal regulations related to MDS and care planning• Excellent communication, organizational, and documentation skills
