As a Part‑Time MDS Coordinator, you will play a vital role in ensuring the well‑being and quality of care for our residents by coordinating the Minimum Data Set (MDS) process.ResponsibilitiesCoordinate the MDS process, including reviewing and updating resident records, conducting assessments, and ensuring accuracy and completenessWork closely with interdisciplinary team members to gather information and complete MDS assessmentsAnalyze and summarize MDS data to identify trends and areas for improvementDevelop and implement plan of care based on MDS assessment and resident goalsCollaborate with healthcare team members to ensure completion of care plans and coordination of servicesMaintain accurate and up‑to‑date documentation and ensure compliance with regulatory requirementsParticipate in quality improvement initiatives and contribute to the improvement of resident care and outcomesRequirementsHigh school diploma or equivalent required; degree in Nursing, Rehabilitation, or related field preferred0 years of experience; prior experience in long‑term care or rehabilitation preferredStrong organizational, communication, and analytical skillsAbility to work independently and as part of a teamFamiliarity with MDS process and software a plusIf you are passionate about delivering exceptional care and committed to serving others, we encourage you to apply for this rewarding opportunity.
