Santa Rosa Post Acute is looking for a detail-oriented and compassionate MDS Coordinator to join our team! If you're an LVN or RN (RN preferred) with a passion for quality care, strong clinical knowledge, and a knack for accuracy and organization, we'd love to meet you.

What You'll Do:

  • Coordinate and complete MDS assessments in accordance with federal and state regulations
  • Ensure accuracy of coding to support appropriate reimbursement
  • Collaborate with interdisciplinary teams to develop and update care plans
  • Monitor documentation and ensure compliance with regulatory standards
  • Participate in care conferences and quality improvement initiatives

What We're Looking For:

  • Active LVN or RN license in California (RN preferred)
  • Experience with MDS 3.0 (preferred)
  • Strong understanding of Medicare/Medicaid guidelines and reimbursement
  • Excellent attention to detail and organizational skills
  • Ability to work collaboratively with nursing and therapy teams
