Position – MDS Coordinator (SNF) Location: Retirement Ranches, Inc. – Clovis, NM (on-site position) Status: Full-Time Schedule: Days (Monday-Friday, 8:00a-5:00p) _____________________________________________________________________ Job Summary: The MDS Coordinator is responsible for coordinating and overseeing the completion of the Minimum Data Set (MDS) assessments in accordance with federal and state regulations, including guidelines established by the Centers for Medicare & Medicaid Services (CMS). This role ensures accurate clinical documentation, appropriate reimbursement under the Patient-Driven Payment Model (PDPM), and compliance with all regulatory requirements within the Skilled Nursing Facility (SNF). The MDS Coordinator collaborates with the interdisciplinary team to assess resident care needs, develop comprehensive care plans, optimize quality outcomes, and support survey readiness.
