    Description

    MDS Coordinator – Uptown Chicago (Edgewater Area)

    We are seeking an experienced and detail-oriented MDS Coordinator to join our team in the vibrant Edgewater neighborhood of Chicago. In this role, you will be responsible for managing the assessment process, ensuring accurate and timely completion of MDS assessments, and coordinating with the interdisciplinary team to develop individualized care plans that meet resident needs and regulatory requirements.

    Key Responsibilities:
    • Oversee and complete the Minimum Data Set (MDS) process in compliance with federal and state guidelines.
    • Collaborate with nursing, therapy, and other departments to ensure accurate documentation and assessment.
    • Monitor and audit records for accuracy and completeness.
    • Participate in care plan meetings and ensure care plans are updated as needed.
    • Stay current with regulatory changes affecting MDS and reimbursement processes.

    Qualifications:
    • Current LPN or RN license in Illinois (required).
    • Previous experience as an MDS Coordinator in a skilled nursing facility.
    • Strong knowledge of RAI process, PPS, and Medicare guidelines.
    • Excellent communication, organization, and time management skills.
    • Proficiency with electronic medical record systems.
