MDS Coordinator

We are seeking a dedicated and detail-oriented MDS Coordinator to join our team at our Westminster, MD location. As a Part-Time MDS Coordinator, you will play a vital role in ensuring the well-being and quality of care for our residents by coordinating the Minimum Data Set (MDS) process.
Responsibilities:
- Coordinate the MDS process, including reviewing and updating resident records, conducting assessments, and ensuring accuracy and completeness
- Work closely with interdisciplinary team members to gather information and complete MDS assessments
- Analyze and summarize MDS data to identify trends and areas for improvement
- Develop and implement plan of care based on MDS assessment and resident goals
- Collaborate with healthcare team members to ensure completion of care plans and coordination of services
- Maintain accurate and up-to-date documentation and ensure compliance with regulatory requirements
- Participate in quality improvement initiatives and contribute to the improvement of resident care and outcomes
Requirements:
- High school diploma or equivalent required; degree in Nursing, Rehabilitation, or related field preferred
- 0 years of experience; prior experience in long-term care or rehabilitation preferred
- Strong organizational, communication, and analytical skills
- Ability to work independently and as part of a team
- Familiarity with MDS process and software a plus
