    Job Description

    The MDS Coordinator is responsible for completing and managing the Minimum Data Set (MDS) process in compliance with federal and state regulations. This role ensures accurate resident assessments, supports appropriate reimbursement under PPS, and leads the development and updates of individualized care plans. The MDS Coordinator also assists with insurance authorization processes and required regulatory notifications.

    Key Responsibilities

    • Complete and submit MDS assessments, CAAs, and related documentation on time and in accordance with CMS guidelines.

    • Develop, implement, and revise individualized resident care plans based on clinical assessments.

    • Coordinate insurance re-determinations, submit required documentation, and communicate with payers on authorizations, denials, and appeals.

    • Support regulatory notifications such as NOMNC and educate residents/families on coverage and appeal rights.

    • Ensure compliance with long-term care regulations and participate in surveys, audits, and QAPI activities.

    • Collaborate with the interdisciplinary team and provide staff education related to MDS and documentation.

    Qualifications

    • Current Nursing License in good standing.

    • Minimum 2 years experience in a skilled nursing facility preferred.

    • Prior MDS experience strongly preferred; RAC-CT/AANAC certification preferred.

    • CPR Certification.

    Required Skills

    • Strong clinical assessment skills and knowledge of MDS 3.0, PDPM, Medicare guidelines, and long-term care regulations.

    • Proficient with EHR systems (MatrixCare preferred).

    Schedule

    Full-time, 40 hours per week with rotating administrative weekend coverage.
