Options for Community Living, Inc. is looking for a Medical Case Manager to join our team! An ideal candidate should meet the following requirements:
- Associate's degree (OR 60 credit hours of college study) in health, human services or education, OR LPN and 2 years of qualifying experience. Bachelor's degree in health or human services (OR RN) preferred, and 1 year of qualifying experience.*
- Valid driver's license, safe driving record, valid auto insurance and access to a vehicle is required.
*Qualifying experience: Verifiable full or part time case management or case work with persons with a history of mental illness, homelessness, or chemical dependence.
An alternative combination of education, credentials, and experience may be substituted for a degree.

Salary Range: $41,860 ($23.00/hr.) - $54,600 ($30.00/hr.)
The above salary range represents Options for Community Living's good faith and reasonable estimate of potential compensation that may be offered to a successful applicant for this position at the time of this job posting and may be modified in the future. When determining a salary offer, several factors may be considered as applicable (e.g., years of relevant experience, education level, language skillset, credentials, professional licensure, budget, and internal equity).
Schedule: Monday - Friday: 9:00 AM - 5:00 PM (35 hours/week)
Location: In person; Ronkonkoma site with field visits required.

Responsibilities:
The Medical Case Manager provides training and assistance to residents of the community residence program (Congregate sites and Apartment sites) in order to help them attain and maintain optimal physical health by coordinating health care planning with residents' health care practitioners.
- Coordinate and be liaison with medical providers for residents in the community residence program, specifically those with additional medical needs.
- Problem-solve to manage acute and chronic medical conditions.
- Liaise with Home Health Aides and coordinate referrals to Home Health care agencies.
- Complete and follow through all steps related to the referral process for in-home care services.
- Arrange for group visits by certain providers who do house visits (podiatrists, dentists, etc.).
- Work with program staff to develop strategies to meet any special needs that a participant may have.
- Complete referrals for meal services based on medical need.
- Implement safety guidelines.
- Collect data on medically-oriented outcomes.
- Identify health-related training needs for staff and residents.
- Assist with referrals to medical day programs and maintain contact with providers.
- Maintain directory of facilities and establish relationships at such facilities in event of client discharge or acute stay.
- In collaboration with other team members, direct the establishment of short and long term goals related to management of health care.
- Assess need for and implement special accommodations in accordance with Options' Accessibility Plan.
- Provide restorative services related to health objectives and write progress notes. Complete and review service plans related to health objectives
- Perform in-person site visits.
- Travel between sites as needed; coordinate with site supervisors while maintaining alignment with primary supervisory direction.
- Assist with discharge planning for residents requiring housing placement in a medical facility.
- Coordinate healthcare planning with the residents' healthcare providers and ensure implementation of medical recommendations and follow-up care.
- Assist with obtaining prior authorizations from medical insurance companies and assist with any other health insurance related concerns.
- Develop and lead structured, interactive health focused groups (educational sessions and engagement-based activities) to promote a healthy lifestyle.
- Provide direct support and transportation to medical appointments as needed. Assist with any referral and advocacy to ensure the resident understands their medical diagnosis, provider recommendations, testing etc.
- Assist residents in understanding and navigating health insurance benefits, including access to services, referrals and retrieving durable medical equipment.
- Collaborate with site supervisors to ensure coordination on resident health responsibilities and consistent staff follow-up.
- May be required to assist with coverage of caseloads, as needed.
- Other relevant responsibilities as assigned by management.

$41,860—$54,600 USD
