Position Summary: The Medical Case Manager provides comprehensive, patient-centered, and confidential services to individuals living with HIV/AIDS, facilitating access to core medical and support services. These include, but are not limited to, medical, dental, mental health, and substance use services, as well as supportive resources such as legal services, transportation, emergency financial assistance, food access, and housing support. The primary objective is to promote and sustain viral suppression by helping patients remove barriers to care to achieve and maintain an undetectable viral load.

Qualifications, Knowledge, Experience, or Licensure
Demonstrated knowledge AND at least six (6) years of work experience equivalent to one of the following education credentials required (degree not required if work experience meets the same level of education):
Associate's degree in human services, social science or health education PLUS four (4) years of case management experience, or
Bachelor's degree in social work, human services, social science, or health education, or
Master's degree in social work, human services, social science, or health education, or
Current Illinois licensure as licensed practical nurse (LPN), registered nurse (RN), or advanced practice registered nurse (APRN).
Prior experience working with individuals living with HIV/AIDS strongly preferred.
3-5 years of experience in a healthcare or medical setting, preferably in a Federally Qualified Health Center strongly preferred.
Working knowledge of and experience with Electronic Health Record (EHR), and MS office applications.
Bilingual English/Spanish preferred, but not required.

All wages are based on relevant years of experience and/or licensure. The minimum rate is the wage that an individual who has no relevant experience earns. No individual will begin employment earning more than the maximum wage listed, which is expected to occur with 13 or more years of relevant experience.
