Medical Director I Elk Grove, California Full Time Schedule: Monday – Friday | 8:00 AM – 5:00 PM On-Call: Rotating Nights & Weekends The Medical Director I provides comprehensive primary medical care for PACE participants while collaborating with the Interdisciplinary Team (IDT) to develop individualized care plans that promote independence, quality of life, and coordinated care.
This physician also oversees participant care across hospitals, skilled nursing facilities, and long-term care settings while providing clinical leadership and mentorship to Nurse Practitioners and other IDT members. The role offers the opportunity to build long-term relationships with participants in a value-based care environment focused on proactive, preventive medicine.
Key Responsibilities
Perform comprehensive medical assessments for PACE participants.
Develop individualized care plans with the Interdisciplinary Team (IDT).
Provide ongoing primary medical care and continuous clinical oversight.
Coordinate participant admissions and discharges from hospitals, skilled nursing facilities, and long-term care settings.
Conduct routine visits to participants residing in nursing facilities.
Collaborate with community physicians and specialists to ensure coordinated care.
Participate in daily Interdisciplinary Team meetings.
Guide clinical decision-making for complex geriatric patients.
Supervise and support Nurse Practitioners and other clinical team members.
Coordinate specialty referrals and follow-up care.
Participate in rotating night and weekend call coverage.
Maintain compliance with Medicare, Medi-Cal, and PACE regulatory requirements.
