Position Summary
Penda Aiken inc. is a staffing firm located in Brooklyn New York. Our client is a New York State Agency and is seeking a detail-oriented Physician or Family & General Practitioner professional to join their team. This position provides clinical services onsite at a 242-bed skilled nursing and rehab facility, including a 32-bed enhanced memory care unit for residents with cognitive impairment and Alzheimer's disease, located in Chenango County, Hwy 220, Oxford, New York 13830. The Department provides care to a population of Veterans, Veteran Spouses and Gold Star Parents.

The Department is seeking no more than two (2) Physicians, or Family & General Practitioners. These positions are to provide services onsite at least five (5) days a week between the hours of 8:00 AM and 4:30 PM, Monday through Friday. They must also provide on-call medical coverage at all times (24 hours, 7 days a week) in the event there is no provider in the facility.
Primary Responsibilities

  • Deliver high-quality physician support staff services, ensuring standard care delivery to the facility's specialized resident base.

  • Perform routine and as-needed resident evaluations for the facility's population of Veterans, Veteran Spouses, and Gold Star Parents.

  • Manage the overall clinical coordination of care for all assigned residents.

  • Maintain clear and prompt documentation within the facility's electronic medical record system.

  • Actively participate in facility care planning processes to optimize long-term healthcare outcomes.

  • Maintain ongoing staffing presence and availability to fulfill the clinical demands of the Department, uphold continuity of care, and support quality patient outcomes.

  • Work collaboratively with Department administration, nursing leadership, and other healthcare professionals.

  • Maintain consistent, responsive communication and availability across both scheduled onsite hours and designated on-call coverage slots.

  • Fulfill 24/7 on-call medical coverage obligations at all times in the event there is no other provider physically present in the facility.
Minimum Qualifications

  • Education: Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited institution.

  • Licensure: Active and unrestricted license to practice medicine in the State of New York.

  • Schedule & Availability: Ability to work onsite 5 days a week from 8:00 AM to 4:30 PM, Monday through Friday, alongside a commitment to full 24/7 on-call coverage rotations.

  • Core Skills: Proficiency with electronic health record platforms, interdisciplinary care plan collaboration, and care management for geriatric residents or those experiencing cognitive impairments and Alzheimer's disease.
