Entry I Salary: $19.11 per hour
Salary Range: Up to $25.29 per hour, depending on experience, qualifications, and position level.
Candidates with 0-12 months of relevant experience will be hired at the Entry I level with a starting salary of $19.11 per hour. Candidates with more than 12 months of relevant experience may be considered for a higher-level position, with compensation commensurate with their experience and qualifications, up to $25.29 per hour.

The Medical Records/Referrals Coordinator is responsible for maintaining accurate, secure, and confidential patient health information. This position serves as a key liaison between patients, caregivers, healthcare providers, and community resources to ensure timely access to medical, specialty, and supportive services. This role will support continuity of care by coordinating referrals, facilitating communication among providers, and ensuring patients receive appropriate follow-up and navigation services to achieve optimal health outcomes.
Medical Records Management
- Collaborate within a team-based care model to improve patient access, continuity of care, and health outcomes.
- Organize, maintain, and manage patient health information and medical records to ensure accuracy, completeness, and accessibility.
- Review medical records for completeness, accuracy, proper identification, signatures, and regulatory compliance.
- Safeguard patient confidentiality and maintain security of medical records in accordance with HIPAA and organizational policies.
- Manage Electronic Health Records (EHR) systems, including scanning, indexing, uploading, and maintaining patient documentation.
- Process medical records release requests promptly and in accordance with established policies and legal requirements.
- Respond to requests for medical information while adhering to confidentiality standards and established procedures.
- Communicate with physicians and healthcare providers as needed to clarify diagnoses, obtain documentation, or gather additional clinical information.
- Ensure timely filing, tracking, and management of medical documentation and correspondence.
- Perform other duties as assigned.
Referral Coordination and Patient Navigation
- Collaborate within a team-based care model to improve patient access, continuity of care, and health outcomes.
- Serve as a liaison between patients, providers, specialists, and community resources to coordinate referral services and care navigation.
- Coordinate and schedule referral appointments, consultations, laboratory services, diagnostic testing, and specialty care within established service standards.
- Communicate promptly and professionally with patients regarding referral status, appointment scheduling, and follow-up requirements.
- Document referrals and patient interactions accurately within the EHR system to support care coordination and clinical workflow.
- Follow established referral protocols, authorization procedures, and patient advocacy practices.
- Conduct outreach and maintain effective working relationships with physicians, specialty clinics, diagnostic providers, and support service organizations.
- Provide timely follow-up communication with patients and respond to inquiries related to referrals and specialty care coordination.
- Assist patients in navigating healthcare services and accessing appropriate internal and community-based resources.
- Perform other duties as assigned.
Minimum Qualifications:
- High School Diploma or General Education Degree (GED) required.
- Certificate or Associate Degree in Health Information Technology, Medical Records Technology, or related field preferred.
- Minimum of 1-2 years of related healthcare, medical records, referral coordination, or administrative experience preferred, or an equivalent combination of education and experience.
