The Medical Respite Program Coordinator serves as the primary case management for the Medical Respite program. This role is responsible for coordinating client care and ensuring that individuals experiencing homelessness receive safe, supportive, and medically appropriate services. The Coordinator works closely with medical providers, social service agencies, and community partners to facilitate seamless care transitions, improve client outcomes, and support program operations.
POSITION RESPONSIBILTIES
Conduct comprehensive client intake assessments, eligibility screening, and triage including at site and in the community
Develop, implement, and monitor individualized care plans in collaboration with medical, behavioral health, and social service providers
Provide ongoing case management to address medical, behavioral health, social, and housing needs
Coordinate and monitor access to medical care, including scheduling appointments, facilitating transportation, and supporting adherence to treatment and discharge recommendations
Support clients in obtaining identification, benefits (e.g., Medicaid, SSI/SSD), and other essential resources necessary for long-term stability
Maintain accurate, timely, and compliant documentation of all client interactions, service plans, and progress notes in accordance with agency policies, regulatory standards, and funder requirements
Facilitate discharge planning and ensure warm hand-offs to community resources, including permanent housing, primary care, and supportive services
Adhere to Mercy Haven and DOH policies, procedures and regulations
Provide on-call coverage as required to support program operations and respond to client needs or emergencies
All other relevant duties as assigned

PAY RATE: $45,000 per year
