JOB REQUIREMENTS

Experience & Skills

• Master's degree in Social Work required.
• 2 years of experience in an acute care hospital or related setting preferred.
• Experience performing comprehensive patient assessments and discharge planning.
• Experience with multidisciplinary care coordination preferred.
• Experience with electronic medical records required; medium proficiency expected.
• Strong communication and collaboration skills.
• Ability to assess high-risk patients and identify appropriate support services.
• Ability to coordinate complex transitions of care and discharge needs.

License & Certification

• LMSW – Licensed Master Social Worker, New York State Board of Social Work, required upon hire; OR
• LCSW – Licensed Clinical Social Worker, New York State Board of Social Work, required upon hire.
• PRI – Patient Review Instrument Certification (PHTC PRI/Screen NYS) required within 90 days of hire.

JOB DESCRIPTION

Role Overview

The SW Care Manager performs comprehensive assessments of patients upon admission and throughout their hospital stay. The position identifies high-risk patients and evaluates discharge planning needs to ensure patients receive appropriate supportive services while helping prevent readmissions.

The Social Work Care Manager collaborates with patients, families, healthcare professionals, payers, and community resources to coordinate safe and appropriate transitions of care.

Patient Population

• All patient populations.
• Special focus on high-risk patients requiring additional supportive services and complex discharge planning.

Responsibilities

• Complete comprehensive initial assessments upon patient admission.
• Screen patients using established tools to identify high-risk indicators.
• Reassess patient needs throughout the hospital course.
• Perform multidisciplinary care coordination at both the intra-hospital and inter-hospital levels.
• Coordinate transitional planning and discharge needs.
• Provide patient and caregiver education.
• Arrange appropriate aftercare and community-based services.
• Assist with commercial payer involvement as needed.
• Identify and address root causes for high-utilizer patients.
• Communicate and collaborate with physicians, nursing staff, social workers, case managers, and other healthcare professionals.
• Provide relevant patient information to appropriate members of the healthcare team.
• Support departmental goals related to:
  • Length-of-stay reduction
  • Readmission prevention
  • Denial reduction
  • Patient satisfaction improvement
• Maintain accurate and timely documentation according to department requirements.
• Participate in Performance Improvement initiatives.
• Perform other related duties as assigned.

CORE QUALIFICATIONS

• Master's degree in Social Work.
• Active NY LMSW or LCSW upon hire.
• PRI certification within 90 days of hire.
• Approximately 2 years of acute-care hospital or related experience preferred.
• Strong assessment and discharge planning skills.
• Experience coordinating care across multidisciplinary teams.
• Ability to identify high-risk patients and develop appropriate discharge plans.
• Working knowledge of electronic medical records, preferably EPIC.
• Strong communication, organization, and problem-solving skills.
