Agency: Alternate Solutions Home Health Columbus

Schedule: Part-time position, 3 days per week.

Territory: Union, Delaware, Madison, Franklin, Licking, Knox, Fayette, Pickaway, Fairfield, and Hocking Counties.

The Medical Social Worker (MSW) provides patients with access to community resources and ensures the patient’s medical, emotional, and safety needs are met. The MSW follows all legal rules and regulations. The Company adopts the social work scope of practice as outlined in the Administrative rules of the state for its licensed MSW personnel. The MSW is responsible for any and all practices and duties within the scope of practice as outlined by the state. 

KEY RESPONSIBILITIES 

• Conducts assessment of patient needs and identifies any barriers 

• Participates in the development of the plan of care 

• Observes and monitors patient conditions 

• Informs the physician, nursing supervisor, and other essential staff of changes in the client’s condition when necessary 

• Educates team members in understanding significant social and emotional factors related to the health problems of the patient and acts as a consultant to other personnel 

• Assists patient and family to understand, accept, and follow medical recommendations and how to utilize home health care services 

• Connects patients and family to community resources and support 

• Assists with patient financial aid applications 

• Assists in long range planning 

• Counsels the patient and family related to coping, changes, and grief 

• Listens to patient and family members 

• Completes all clinical documentation in accordance with agency protocol and Medicare/Federal guidelines 

• Documents all aspects of care, assessments, and patient education 

• Maintains active social work license in state of employment 

• Completes chart audits at agency location as requested 

• Communicates with scheduler any changes outside the normal 

• Participates in all on-call requirements and case conferences 

• Attends in-service trainings and mandatory agency meetings 

• Maintains a professional appearance as a representative of the company 

• Understands and practices agency policies and procedures 

• Drives to patient’s primary location per scheduled visit. Daily attendance at assigned visit locations. Documents all aspects of subsequent, discharge, eval/recert/resumption of care visits within 24 hours of visits. Completes and submits all required documentation within specified company requirements. 

• Current applicable license as a Medical Social Worker. Follows plan of care as permitted within the scope of practice for a Licensed Medical Social Worker (R 338.2901). 

• Fulfills MSW visits in assigned geographic location per patient need within federal and state specifications. 

• Dexterity & vision to complete documentation on a PDA. Driving, as needed. 

• Other duties as assigned. 

QUALIFICATIONS 

• One year of Medical Social Work experience required. 

• Ability to create positive impressions and communicate with a variety of people. 

• Maintain effective communication with patients, families, physicians and co-workers. 

• Ability to remain calm, have patience and be accommodating. 

• Compassionate and caring while working with patients. 

• Knowledgeable on social work best practices. 

• Ability to make appropriate judgments. 

• Ability to identify a situation and handle it with the best possible solution. Detail-oriented and observant. 

• Disciplined style of work ethic. Ability to prioritize and be timely. Works efficiently. 

EDUCATION AND CREDENTIALS 

• Licensed Social Worker with current license in the state of employment. 

• Master of Social Work required.
