    General Summary/Responsibilities:

    The Medical Social Worker is responsible for providing social work services to patients in their homes per physician's plan of care.

    Minimum Level of Experience Required: 1-3 years of job related experience3 - 5 years of job related experience

    Prior job title or occupational experience: One year experience in a health care setting (hospital, clinic rehabilitation center, home care, etc.3-5 years of experience providing clinical services; experience in home care or community/outpatient setting preferred.
