Medical Social Worker – Home Health (Per Diem)
Southwoods Health is hiring a Per Diem Medical Social Worker for our Home Health division. In this role, you will provide essential medical social services directly in patients' homes, including implementing care plans, delivering interventions, and educating patients and their families.
 
Key Responsibilities:
  • Plan and deliver safe, effective social work care tailored to individual patient needs.
  • Complete accurate documentation regarding patient status and responses to interventions.
  • Educate patients and their families on available resources, coping strategies, and care plans.
  • Collaborate with the multidisciplinary healthcare team to provide seamless care coordination.
  • Protect patient privacy and maintain strict confidentiality of medical records.
  • Utilize community and clinical resources efficiently to manage healthcare costs.
  • Maintain full compliance with all local, state, and federal regulatory agencies (e.g., OSHA, ODH, TJC).
Qualifications:
  • Licensed Social Worker (LSW or LISW) with an active license in Ohio.
  • Master’s Degree in Social Work (MSW).
  • 1+ years of experience in the healthcare field.
  • Strong communication, prioritization, and time management skills.
  • Excellent critical thinking and clinical problem-solving abilities.
