Medical Social Worker II

Summary The Medical Social Worker (MSW) in Hospice and Palliative Care assesses patient/family needs and coping skills that support the patients unique cultural and spiritual beliefs. Provides holistic patient/family centered care across the treatment settings. Provides education, interventions and community resources specific to the patient/family need. Is a member of the patients multidisciplinary care team. Participates in discharge planning and treatment planning as needed. Conducts family meetings and advance care plan conversations. Coordinates with the care team and other coworkers to facilitate the patients goals and achievement of quality and cost-effective outcomes. Education Masters Degree in Social Work. Licensure/Certification Must have current Indiana LSW or LCSW license. Must have valid drivers license and proof of insurance. Experience Minimum of one (1) years experience in health care with experience in hospice or palliative care preferred. Other Qualifications Demonstrates thorough working knowledge of social work process, community agencies, services, and resources available to assist patients and families in the resolution of emotional , social, and financial difficulties related to change in patients health status. Requires general knowledge of the normal growth, development, related behaviors, needs, comfort, and range of treatment appropriate to the age of the patient and diagnosis to support the provision of an individualized plan of care. Requires sound analytical and organizational skill to assess patients needs, plan and coordinate appropriate therapeutic intervention and to manage case load to achieve efficiency and productivity. Must have good verbal and written communication skill.
