Job Summary and QualificationsThe Medical Social Worker LCSW provides clinical counseling and consulting services to patients, families and staff members and assists in facilitating the development of new and expanded social work services and programs within the hospital and plans for discharge and aftercare. If that is you, come be a part of what makes us great and apply today!You will be interviewing patients and family members to obtain relevant bio-psychosocial information and determines the effect of illness and trauma upon patient and patient's family; identifies complex, multiple, social, emotional, financial and environmental factors which contribute to the illness or interferes with the patient's ability to benefit medical careYou will be developing and implementing a social work treatment plan for patients and family members based on an analysis of patient's psychosocial factors and medial illness. This may include discharge planning and insurance utilization review on appropriate unitsYou will be utilizing appropriate treatment modalities, including individual, family and group intervention. Provides counseling to patients and families in areas such as family problems, coping with illness, trauma, loss of function and griefYou will be participating as part of the multidisciplinary team in case and family conferences to assist in formulating overall patient care plans, interprets psychosocial aspects of the patient's illness to other staffYou will be providing community and professional staff training regarding patient psychosocial needs and/or coping with illness, grief and lossYou will be providing evaluation, support and follow-up referrals, planning and reports as needed by all victims of child abuse, rape, elder abuse and family violenceYou will be providing evaluation and referral services for patients who are alcohol or drug dependentWhat qualifications you will need:LCSW license requiredMaster of Social Work (MSW) required1 year acute care hospital experience highly preferred Benefits Riverside Community Hospital offers a total rewards package that supports the health, life, career and retirement of our colleagues.
