Position Overview
A respected acute care hospital in Burbank, California is seeking an experienced and compassionate Medical Social Worker (MSW) for a 13-week contract assignment. This role is ideal for a professional with a strong background in acute care social work and discharge planning who is committed to supporting patients and families through complex healthcare transitions.
The Medical Social Worker will collaborate closely with interdisciplinary healthcare teams to ensure safe, effective, and patient-centered continuity of care.
Key Responsibilities
- Conduct psychosocial assessments for patients and families
- Coordinate discharge planning and post-acute care services
- Collaborate with physicians, nurses, case managers, and interdisciplinary teams
- Assist patients with access to community resources, financial assistance, and support programs
- Provide crisis intervention, emotional support, and counseling as needed
- Advocate for patient needs and continuity of care throughout hospitalization
- Maintain timely and accurate documentation in compliance with facility standards
- Participate in patient care planning meetings and educational initiatives
- Ensure adherence to hospital policies, regulatory requirements, and social work standards
Qualifications
- Master’s Degree in Social Work (MSW) required
- Minimum 2 years of acute care medical social work experience required
- Previous discharge planning experience required
- Active California Social Work License required
- Pending California license accepted, but must be active prior to start date
- First-time contract professionals accepted with successful screening
- Strong communication, assessment, crisis management, and care coordination skills required
