Responsibilities:

• Assess the social and emotional factors related to the client’s health status
• Develop and implement a social service care plan
• Collaborate with other healthcare professionals to ensure quality care
• Maintain accurate and timely documentation of services
• Provide counseling and emotional support to patients and families.
• Facilitate support groups and educational sessions.
• Connect patients with community resources and support services.
• Collaborate with external agencies and organizations.
• Advocate for patients' rights.
• Participate in care conferences and collaborate with the healthcare team.
• Maintain accurate and timely documentation of patient interactions

Requirements:

• Licensed Medical Social Worker in the state of Florida
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a team
• Knowledge of medical terminology and healthcare regulations
• Ability to work with a diverse population
