Medical Social Worker (MSW) – Case Management | Contract Assignment

Location: Burbank, California 91505
Shift: Day Shift | 5x8 Hours (08:00 AM – 04:30 PM)
Duration: 13 Weeks
Weekly Hours: 40 Hours

Job Summary

A leading acute care hospital in California is seeking an experienced Medical Social Worker (MSW) for a 13-week contract assignment. This role is ideal for a compassionate and detail-oriented professional with strong experience in hospital-based social work and discharge planning.

The Medical Social Worker will work closely with interdisciplinary healthcare teams to support patient care coordination, psychosocial assessments, discharge planning, and community resource referrals to ensure safe and effective transitions of care.

Key Responsibilities

  • Conduct comprehensive psychosocial assessments for patients and families
  • Provide crisis intervention, counseling, and emotional support services
  • Develop and coordinate safe discharge plans with physicians, nurses, and care management teams
  • Assist patients and families with community resources, financial assistance, placement services, and post-acute care planning
  • Coordinate referrals for rehabilitation, skilled nursing facilities, hospice, home health, and behavioral health services
  • Advocate for patient rights and continuity of care throughout hospitalization
  • Maintain accurate documentation within EMR systems
  • Collaborate with interdisciplinary teams to improve patient outcomes and reduce readmissions
  • Ensure compliance with hospital policies, HIPAA regulations, and state/federal guidelines
  • Participate in multidisciplinary rounds, care conferences, and team meetings

Requirements

  • Master’s Degree in Social Work (MSW) required
  • Active California Social Work License required by start date
  • Pending California license accepted prior to start
  • Minimum 2 years of acute care hospital social work experience required
  • Experience with both medical social work and discharge planning required
  • Strong communication, crisis intervention, and care coordination skills
  • First-time travelers welcome (manager screening required)
