s aMedical Social Worker(MSW), you will:

• Completecomprehensive patient assessments.
• Participate in developingindividualized Plans ofCare.
• Identify barriers tocare and recommendappropriate interventions.
• Connect patients andfamilies with valuablecommunity resources.
• Assist with financialassistance applicationsand long-termplanning.
• Educate and supportpatients and caregiversthroughout theircare journey.
• Provide counseling relatedto adjustment, coping, loss, and grief.
• Collaborate with physicians, nurses, therapists, and the interdisciplinarycare team.
• Complete accurate clinicaldocumentation in accordancewith agency, Medicare, and federal guidelines.
• Observe,document, andcommunicate changes inpatient condition.
• Attend required trainingand agency meetings.

Qualifications

• CurrentMedical Social Worker(MSW)license in thestate of employment(limitedlicenses not accepted)
• Master's Degree inSocial Work (MSW) required
• Minimum of oneyear ofMSWexperience in anacute care setting
• Home health experiencepreferred
• Valid driver's licenseand current autoinsurance
• Ability to meetthe physical demandsof the position
