We are seeking a compassionate, organized, and team-oriented Full-Time Medical Social Worker (MSW) to serve patients in Walnut Creek and surrounding communities.

Responsibilities

• Conduct psychosocial assessments during patient admission
• Evaluate social, emotional, and environmental factors impacting care
• Collaborate on interdisciplinary care plans
• Provide counseling, resource linkage, and discharge planning support
• Consult with physicians and care teams regarding patient needs and care adjustments
• Maintain timely and accurate clinical documentation
• Supervise Social Work Assistants as needed

Requirements

• Current California Licensed Clinical Social Worker (LCSW) license
• Valid California Driver’s License
• Reliable transportation for home visits
• CPR and First Aid certification
• Ability to travel throughout Walnut Creek and surrounding East Bay areas
