    Salary: PTEA Union Rate (V18) $34.76

    Job Details:

    • This is a grant-funded position
    • This is a fulltime onsite position
    • This position requires a background check prior to commencement of service.

    Position Summary:

    The Neurodegenerative Disease Program Social Worker will interview clients and/or their families to assess the psychosocial situation and determine needs. The Social Worker will collaborate with the multidisciplinary team both within Temple Health and within the community in developing plans to improve the clients''/families' well-being and help clients/families connect with vital and supportive resources. The Social Worker will also have the opportunity to be involved in outreach and educational projects for the Neurodegenerative Disease Program.

    Required Education and Experience:

    • Master's degree from an accredited School of Social Work. At least two years of related social work experience.
    • An equivalent combination of education and experience may be considered.

    Responsibilities:

    Assistance provided will include, but will not be limited to,

    • Insurance/Medicare/Medicaid issues, disability, unemployment,
    • Registering with government agencies, obtaining benefits, completing forms,
    • Writing letters of medical necessity
    • Connecting to community resources,
    • Offering emotional care when needed and facilitating support groups.
    • This person will provide follow-up communication to ensure clients' needs are being met. *The incumbent will conduct regular consultation with his/her supervisor,
    • Prepare and maintain required documentation,
    • Participate in meetings within the department
    • Perform other duties as assigned.

    Required Skills and Abilities:

    • Demonstrated computer skills, including proficiency with word processing.
    • Strong written and verbal communication skills.
    • Demonstrated interpersonal skills, along with the ability to interact with a diverse group of patients, families, staff, faculty and administration.
    • Demonstrated organizational skills, along with the ability to prioritize tasks and be detail-oriented in a high-volume priority-driven environment.
    • Demonstrated analytical and problem-solving skills, with strong attention to detail.
    • Demonstrated ability to work independently or as part of a team.

    Preferred Skills and Abilities:

    • Bilingual
    • Experience with neurodegenerative diseases
