A Medical Social Worker provides social work services to patients on an intermittent basis in their place of residence while under the care of Seasons Home Health Care. · Plans/coordinates all social services within Residence. · Participates in the development of the plan of care. · Participates in patient care conferences. · Maintains current knowledge of Medicare Conditions of Participation for social work services. · Develops a cooperative relationship and communicates effectively and professionally with physicians. · Participates in the coordination of All Seasons' services with the services of other community resources; uses community resources. · Serves as a resource person to employees, patients, physicians and other allied healthcare providers. · Attend meetings, CEUs on a regular basis
