Are you a compassionate and dedicated Licensed Social Worker looking to make a meaningful impact in the lives of patients in their homes? We are seeking a qualified individual to join our home health care team, providing vital mental health services to individuals in their home.
Key Responsibilities:
- Conduct psychosocial assessments for patients and families, identifying needs, barriers, and resources
- Develop and implement care plans addressing emotional, social, and practical needs
- Provide counseling, crisis intervention, and patient/family education as appropriate
- Collaborate with clinical, intake, and administrative teams to coordinate services and referrals
- Assist with discharge planning, patient advocacy, and connecting patients to community resources
- Maintain accurate EMR documentation of assessments, interventions, and outcomes
- Monitor patient progress and report relevant information to clinical leadership
- Participate in quality improvement initiatives and case reviews
- Perform other administrative or clinical duties as assigned
Position Qualifications:
- Master’s degree in Social Work (MSW) required; LCSW preferred
- Current state licensure as a Social Worker (or eligibility)
- 2–3 years healthcare or home health experience preferred
- Strong assessment, counseling, and communication skills
- Knowledge of community resources, patient advocacy, and care coordination
- Proficiency with Microsoft Office; experience with EMR systems preferred
- Ability to work independently in a fast-paced environment
- Availability during business hours with occasional after-hours/weekend coverage
Hours: PRN, Pay-per-visit
Work Type: In-person / traveling within a reasonable distance to patients' homes.
Pay: $40 - $45 per hour.
