Position Overview

Working under the direction of the LPN Supervisor, you'll partner with nurses and other healthcare professionals to provide high-quality care for our senior residents. This role includes assisting residents with medication self-administration, monitoring changes in their condition, maintaining accurate records, and helping create a safe, welcoming community where residents can thrive.

Primary Responsibilities

  • Deliver resident-focused care that reflects Lutheran Haven's mission, values, and commitment to excellence
  • Build respectful, positive relationships with residents, families, and fellow team members
  • Assist and supervise residents with the safe self-administration of medications in accordance with established procedures
  • Ensure medications are properly prepared, available, and documented
  • Observe residents for physical, emotional, or behavioral changes and promptly communicate concerns to the nursing team
  • Measure and document vital signs, weights, and other required health information
  • Coordinate transportation and assist residents with appointments outside the community when needed
  • Maintain complete, accurate, and timely resident documentation and medication records
  • Follow infection prevention protocols, safety procedures, and universal precautions at all times
  • Encourage residents to participate in social, recreational, and wellness activities that enhance their quality of life
  • Participate in required meetings, in-service education, and ongoing training opportunities
  • Perform additional responsibilities as assigned to support quality resident care

Qualifications

  • High school diploma or equivalent preferred.
  • Medication Administration Certificate is preferred but not required.
  • Paid training and certification available for qualified candidates.
  • Current CPR certification preferred or ability to obtain after hire.
  • Basic computer skills.
  • Strong communication and interpersonal skills.
  • Ability to exercise sound judgment and maintain professionalism in a healthcare environment.
  • Reliable, compassionate, and committed to providing excellent resident care.
