Memory Care Clinical Coordinator (RN/LPN) — $15,000 New Hire Bonus! Position Summary: Wake Robin is seeking a compassionate, detail-oriented Memory Care Clinical Coordinator (RN or LPN) to play a vital role in ensuring smooth day-to-day clinical operations and resident-centered care in our Linden Health Center. As a member of the Nurse Leadership Team, this position bridges communication between residents, families, staff, and providers to enhance care outcomes and support our community’s mission of wellness and dignity. Our 18-home Memory Care neighborhood is thoughtfully designed to support individuals living with Alzheimer's disease and other forms of dementia. Grounded in compassion, dignity, and safety, it provides a secure, welcoming environment where residents thrive through meaningful relationships, personalized care, and engaging daily experiences. Key Responsibilities: Clinical & Administrative Duties - Perform clinical tasks such as blood draws, IV starts, x-ray ordering, and medication administration.
- Maintain medication and supply rooms (organization, cleanliness, and expiration checks).
- Update care plans and manage secure communications.
- Support staff with evaluations, forms, vitals, weights, notifications, admissions/discharges, and documentation.
- Oversee daily workflows for all shifts and participate in weekend call rotation.
Resident Admissions & Transitions - Support communication with hospitals and providers for new resident admissions.
- Coordinate preparation of resident homes and hospice referrals.
Resident & Family Support - Serve as a key point of contact for residents and families via phone, email, and in-person meetings.
- Manage televideo provider visit scheduling and maintain the resident appointment calendar.
Staff Support & Coordination - Provide ongoing support to nursing staff throughout shifts.
- Support onboarding and training of new Licensed Nurse Assistants and Nurses.
- Coordinate LNA group assignments and shift transitions.
- Act as liaison with the Wellness Clinic team (MDs, DNP, RN, Medical Assistant).
- Schedule medical and lab appointments with external providers.
Meetings & Collaboration - Attend and participate in Interdisciplinary Team Meetings, Care Conferences, and Resident Council meetings to ensure consistent communication and follow-through on care plans and resident needs.
Qualifications: - Current Registered (RN) or Licensed Practical Nurse (LPN)
- Meaningful experience in long-term care and dementia care, demonstrating a clear interest in furthering a career in this setting
- Shows initiative and curiosity, with a genuine desire to learn, grow, and contribute to a resident-centered community in a leadership role
- Collaborative, team-oriented approach with the ability to build positive working relationships
- Strong organizational and communication skills, with the ability to prioritize effectively
Schedule: - Primarily Monday through Friday, 8:00 am – 5:00 pm with flexibility to support occasional evenings, nights or weekends based on training or operational needs
- Weekend on-call responsibilities are shared on a rotating basis among the nursing leadership team, approximately every 4 weekends
Salary - $81,000–$101,000 annually for RNs
- $67,000–$81,000 for LPNs.
- Eligible for a $15,000 new hire bonus
