Memory Support Program Manager

Rydal Park, a premier HumanGood Life Plan Community, is seeking a compassionate and highly motivated Memory Support Program Manager to lead our Memory Support neighborhood. This leadership role oversees the resident experience, team member development, dementia programming, family support, and day-to-day operations of the neighborhood.

The Memory Support Program Manager serves as a champion of HumanGoods Immerse philosophy (person-centered dementia care), working closely with residents, families, nursing, rehabilitation, social services, life enrichment, and other interdisciplinary team members to create a meaningful, engaging, and supportive environment for those living with memory loss.

We welcome candidates from a variety of professional backgrounds, including Licensed Practical Nurses (LPNs), Social Workers, and Certified Therapeutic Recreation Specialists (CTRS) who bring experience supporting individuals living with dementia and leading teams in healthcare settings.

** Full-time, Monday - Friday, 8:00 am - 4:30 pm   ** Salary: based on licensure and experience 

Key Responsibilities include, but may not be limited to:

• Lead and promote the HumanGoods Immerse Program, a person-centered dementia care philosophy throughout the community. 
• Supervise, coach, mentor, and develop team members while fostering a culture of accountability, engagement, and compassion.
• Partner with nursing, life enrichment, rehabilitation, dining, and social services to ensure individualized resident support plans are implemented effectively.
• Provide education, guidance, and support to residents' families as they navigate the dementia journey.
• Serve as a resource to staff regarding dementia care approaches, behavior support strategies, communication techniques, and resident engagement.
• Monitor resident well-being and identify opportunities to enhance quality of life, engagement, and satisfaction.
• Support the development and implementation of meaningful programming tailored to residents' interests, abilities, and cognitive needs.
• Participate in care conferences, interdisciplinary meetings, and family meetings.
• Ensure regulatory compliance, accurate documentation, and adherence to community policies and procedures.
• Respond appropriately to resident, family, and operational concerns and assist in resolving issues collaboratively.
• For licensed clinical candidates, responsibilities may also include functions performed within the scope of professional licensure.

Qualifications

• Current Pennsylvania LPN license, Bachelor's or Master's degree in Social Work, or CTRS certification preferred.
• Minimum 2 years of experience working with older adults living with dementia, Alzheimer's disease, or related cognitive impairments.
• Prior supervisory, management, or team leadership experience in a healthcare, long-term care, rehabilitation, memory care, or senior living setting strongly preferred.
• Demonstrated ability to educate, coach, and support families and staff regarding dementia care best practices.
• Strong communication, relationship-building, conflict-resolution, and problem-solving skills.
• Ability to collaborate effectively with residents, families, physicians, and interdisciplinary team members.
• Experience with electronic health records and documentation systems; PCC experience preferred.
• CDP, CDCM, or other dementia-related certifications are a plus.
