Good Landing Recovery is seeking a compassionate, organized, and motivated Mental Health Case Manager to join our team. In this role, you will provide comprehensive case management services to individuals receiving treatment for mental health and co-occurring substance use disorders. You will serve as an advocate for clients, helping them access resources, coordinate care, and develop the skills needed to achieve long-term recovery and independence.

Key Responsibilities
- Conduct comprehensive client assessments to identify needs, strengths, and barriers to recovery.
- Develop, implement, and monitor individualized treatment and case management plans.
- Coordinate care with therapists, psychiatrists, medical providers, family members, and community partners.
- Assist clients in accessing community resources, including housing, transportation, employment, education, financial assistance, and healthcare services.
- Provide crisis intervention and appropriate referrals when necessary.
- Maintain accurate, timely, and compliant documentation in the electronic health record (EHR).
- Monitor client progress and update treatment goals as needed.
- Educate clients and families on mental health conditions, recovery strategies, and available support services.
- Advocate for clients to ensure continuity of care and successful transitions between levels of treatment.
- Participate in interdisciplinary treatment team meetings and contribute to collaborative care planning.
- Ensure compliance with HIPAA, state regulations, and organizational policies.
Qualifications
Required
- Bachelor's degree in Social Work, Psychology, Human Services, Counseling, or a related field.
- Minimum of one (1) year of case management or behavioral health experience (preferred).
- Knowledge of mental health diagnoses, substance use disorders, and community resources.
- Strong communication, organizational, and problem-solving skills.
- Ability to work effectively with diverse populations.
- Proficiency with electronic health records (EHR) and Microsoft Office.
- Valid driver's license and reliable transportation (if community-based services are required).
Preferred
- Certified Case Manager (CCM) or Targeted Case Management certification.
- Experience working in substance use treatment, residential treatment, or outpatient behavioral health settings.
- Knowledge of trauma-informed care and motivational interviewing techniques.
- CPR/First Aid certification.
