Job Title: Mental Health Intake Coordinator
Entity: Hospital of the University of Pennsylvania Cedar (HUP Cedar)
Department: Emergency Psych Crisis / CRC
Location: 501 S. 54th Street (54th & Cedar)
Hours: 0.01 FTE (24 hours per four-week schedule block), Day shift (7a - 7:30p as needed)
Summary:
- Under the supervision of the Nurse Manager, provides emergency, psychosocial evaluations and services to patients and their families in the Hospital of the University of Pennsylvania Crisis Response Center.
Responsibilities:
- Patient Care Activities Provides activities of daily living to assigned patients. Gives complete or partial bath or assists patient with shower. Prepares patient for meals and/or feeding as needed. Toilets patients per bed pan, urinal, or bathroom. Assists patients in ambulating, transferring and transporting. Collects non-invasive urine, stool and sputum specimens. Takes vital signs in an accurate and precise manner and records on appropriate form. Informs RN and/or doctors of vital signs that deviate from normal standards. Orients new patients to surrounding and unit procedures. Performs searches as per unit policy and procedure. Understands the purpose of incident reports and utilizes accordingly. Initiates and/or assists in CPR when indicated. Participates in secluding and restraining patients as per policy and procedure. Designs and implements emergency psychosocial plans of care. Uses crisis intervention counseling techniques to reduce the escalation of a crisis situation. Conducts and completes documentation of initial intake reviews. Evaluates client's response to care and modifies approaches as needed. Provides emotional support and enables patient and family to participate in treatment plan. Collaborates with other Psychiatric Crisis Service professionals concerning the needs of the patients and the resources available to meet these needs. Interprets programs and regulations of community facilities and resources to patients, physicians, and/or family members. Maintains familiarity with the Mental Health Laws and their implementation. Collaborates with other agencies to provide continuity of care for clients and to facilitate therapeutic follow-up care. Documents in progress notes in a timely and accurate record of clients care while in the Crisis Unit. Performs other related duties as assigned by immediate supervisor.
- Crisis Management job specific duties: Assists patients to establish goals. Recognizes signs of potentially dangerous situations. Uses de-escalation techniques. Uses limit setting and redirection. Maintains appropriate boundaries. Is aware of Psychiatric Emergency Assistance Team Procedures. Is aware of psychosocial stressors and precipitating factors. Recognizes loss and grief issues. Is able to evaluate support systems. Assesses suicide risk factors. Assesses homicide risk factors. Assesses potential for violence. Recognizes signs of drug and alcohol abuse. Able to recognize and intervene with anxiety symptoms. Recognizes acute intoxication. Assesses coping skills Understands psychological responses to disasters or trauma. Obtains data for psychiatric and medical history. Conducts telephone triage. Arranges and coordinates inpatient admissions. Arranges and coordinates outpatient referrals Provides guidance for 302 petitioners. Formulates report for psychiatrist. Participates in insurance verifications. Performs and assists with pre-certifications. Arranges appropriate transportation. Assists with billing procedures. Coordinates patient transfers to and from other. Interacts with police and emergency personnel. Understands divert procedures. Assists patients with meals. Aware of community resources. Able to complete computerized registration of patients. Interacts with county delegate to present 302 Involuntary Commitment petitions.
Education or Equivalent Experience:
- H.S. Diploma/GED (Required)
- Equivalent Experience: And 2+ years, 12 semester credit hours in sociology, social welfare, psychology, gerontology or other social science and 2 years of experience in public or private human services with 1 year of mental health direct care experience.
- Bachelor of Arts or Science preferred
- Education Specialization: Social Work, Psychology, Marriage and Family Therapy, Counseling or any Healthcare related field
- Master of Arts or Science preferred
- Education Specialization: Social Work, Psychology, Marriage and Family Therapy, Counseling or any Healthcare related field
